Vital Records

Office of the Town Clerk All requests for Vital Statistic information must be received in writing, accompanied by the proper payment. Certified copies (with a raised seal of authenticity) are available at a cost of $5.00 each. Please complete the appropriate request form and mail it with your check, payable to “The City of Stamford”. Photo ID must be shown when requesting information. For mailed requests, a photocopy of ID must be included. • Copy of Birth Certificate Form • Copy of Death Certificate Form • Copy of Marriage Certificate Form • Marriage License Application Form Requests for vital statistic information, over 100 years old, are considered genealogical. The City of Stamford is unable to fulfill requests for this information. City historic records are open for public inspection and anyone welcome to visit the office and search their family’s history.
