ALL APPLICANTS MUST APPEAR IN PERSON, INCLUDING CHILDREN
Required Documents:
Adults (Ages 16 & over):
- Two passport photos (photo services available at the Library for a fee of $12).
- Original Birth Certificate (with raised seal) OR Naturalization Certificate OR old passport.
- Valid Driver's License.(Copy of front and back)
- Personal or bank checks (no cash accepted).
- Proof of name change (Marriage Certificate, court order for name change or divorce decree).
Ages 16 - 18:
- Original Birth Certificate with raised seal OR old Passport OR Naturalization Certificate.
- One parent must accompany minor to supply ID and signature. If parent is unavailable, applicant must bring written parental permission.
Children 15 and younger:
- Original Birth Certificate with raised seal AND old Passport AND Naturalization Certificate.
- Both parents/legal guardian must be present or submit a NOTARIZED written consent statement by non-appearing parent.

New Passport Cards:
Passport cards are a less expensive form of identification that will be accepted at land and sea ports-of-entry for travelers arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air.
